Person centred Tailored Support

This comprises a 2 hour face to face session, allowing the consultant plenty of time to get to know your care needs and allow you the time to fully engage with what you may be looking for. This consultation is an excellent way to establish what your ongoing care needs may look like, and receive support to better understand the care sector itself.

During your session the consultant will cover the following elements:

  • Comprehensive overview of care sector options including a package of visuals to demonstrate information being provided.

  • Support with understanding person specific needs and how to ensure these are considered when looking for care.

  • A completed tailored support plan identifying what care needs are required. This will cover all aspects of care considered by providers including, but not limited to, your needs and wants for personal care, preparation of meals, medication and condition support, and keeping you as safe as possible in the right environment.

  • (You will receive a digitally produced care plan after your session once compiled remotely).

  • As a result of this session you will leave feeling better informed as to your options, have increased understanding as to what you may be looking for, and recognise the best way to source and secure the care option that is right for you.

  • You will have a completed care plan documenting needs as captured on the date of the session. This document will allow you to reflect on all aspects of the care you may be looking for and can encourage active and appropriate discussions with care providers.

  • Complete visibility to care needs through tailored questioning.

  • Professional support and guidance to navigate care sector.

  • Unbiased and objective view to care needs.

Face to face consultations can take place in many environments including home environments, hospitals and care facilities. We recommend that consultations do not take place in busy public settings due to the sensitive nature of conversations. Where you would like us to visit you in a public setting we work to protect you as far as possible through suggestion of alterations to the environment, or follow up phone calls, so that we can ensure you feel safe to speak openly. It is also worth considering if you would like for the consultant to assess environment as part of your assessment (ie to consider suitability and practicality of receiving care at home). Please take the time to consider where you wish for the assessment to take place. Our consultants are also trained to assess mental capacity as part of the consultation but would need the individual to be present for discussion in this instance.

This service is £500 and includes both face to face and remote components.

If you are looking for additional support with understanding how the care sector works, and what to consider when sourcing care, we can also complete our popular “Understanding Care needs and sourcing appropriately” conversation for an additional £150 (This will require an additional 1 hour of your time).

Please note that prices do not include consideration for costs incurred when travelling. Our team will be happy to let you know if there would be any additional charge to services based on consultant travel and, where applicable, overnight accommodation.
